New Patients

You can register with the Practice if you live within our Practice area and are a permanent UK resident. Our Practice area covers all of Bedlington, Choppington, and Guidepost. We also cover, Hepscott, Stannington and some parts of Morpeth. If you are unsure if you reside in our Practice area please ask one of our Receptionists.

How to register

Please complete the forms below and bring to the surgery along with your identification (proof of ID and address. We do not need identification for children as long as at least one parent is registering with us. Examples of proofs of identity: Passport, Driving Licence, Medical Card, Birth Certificate. Examples of proofs of address: Utility Bill, Bank statement, Rent Book or Council tax statement. A more detailed list can be found in the New Patient Registration Form/Questionnaire below.

NOTE – We can only accept registrations between the hours of 8:00am and 16:30pm unless pre-arranged with the surgery (Tel: 01670 822695).

All new patients are invited to make an appointment with one of our practice nurses to discuss any health issues. The nurse will also check your blood pressure.

All patients will have a named GP who will have overall responsibility for the care and support provided to them. This does not prevent you from seeing any GP within the Practice as you currently do, nor does it change the GP you prefer to see.

Moving house / change of address

If you are moving house, please check that your new home is still within our practice area. If this is not the case, you will need to make arrangements to find a new GP.
