New Patients

You can register with the Practice if you live within our Practice area and are a permanent UK resident. Our Practice area covers all of Bedlington, Choppington, and Guidepost. We also cover, Hepscott, Stannington and some parts of Morpeth. If you are unsure if you reside in our Practice area please ask one of our Receptionists.

How to register

Please call in at the surgery to collect a new patient pack. Alternatively please print off and complete the following documents and hand them in at Reception with your identification.

New Patient Registration Form (GMS1)

Patient Form – New Patient Questionnaire

All new patients are invited to make an appointment with one of our practice nurses to discuss any health issues. The nurse will also check your blood pressure.

All patients will have a named GP who will have overall responsibility for the care and support provided to them.

This does not prevent you from seeing any GP within the Practice as you currently do, nor does it change the GP you prefer to see.

If you would like to know who your named GP is please ask a member of the Reception Team when you next contact the surgery.

If you wish to change your named GP, please contact the Practice and we will make reasonable efforts to accommodate your request.

Moving house / change of address

If you are moving house, please check that your new home is still within our practice area. If this is not the case, you will need to make arrangements to find a new GP.
